Hello Dr Park, I am 59-year-old male and an OSA sufferer. When I was 8-year-old, I had developed a boil or some infection of that sort. My grandma had removed pus from inside the nose by pressing from up with her fingers which resulted in permanent depression. I can recollect now that from then onward my memory had started deteriorating: initially, short-term, and, presently, long-term memory, too.. These days, I use auto-cpap machine and maintain AHI from 3-5. However, when I am awakened (which I do 4-5 times/night) I find my heart racing.
From around 1985 till 2005, I had added weight that went upto 67 kg. I had slight diabetes (upto 160mg/dl) and high BP (upto 150/92 mm of Hg), tachycardia, high RBC and Hb% etc. Then, I changed my food and life style.
Presently, my height and weight are 162 cm and 59-60 kg, respectively. My BP remains mostly at 110/70 with no tachycardia. My visceral fat is just 8%. I look slim without a thickened neck or folds under chin which I never had. The thickness of my subcutaneous belly-fat is under 1 cm. Last almost 20 years, I have maintained discipline regarding my food and drinks. However, my fasting blood glucose remains around 120 mg/dl despite the fact that I do not take sugar and sugary foods and seldom take a source of carbohydrate that has glycemic index above 35-40. I do not take refined oils, rice, wheat flour, manufactured packaged foods and drinks. I take my food and drinks prepared under the boiling point of water. I take mustard oil and unheated olive pomace oil, only. My foods and drinks are minimally disturbed from their natural states.
Twice I had gone for polysomnography. However, both the times I could not sleep. I am an early sleeper and can not sleep with sound or other disturbances around and expecting something or someone. In both occasions, the technicians came late. I could not fall asleep. Hence, titration was not possible in both the times. Hence, I had to self-titrate with trials and errors
I have procured my 3rd cpap machine, presently, which is an AUTO from Devilbiss, USA. My present setting is at 8.0-11.0.
On 24 October, this year, I had a lifetime record AHI of 0.0 with great increase in my energy and other health parameters, the following day. However, on 25th, AHI was 4.5.
Would you please suggest the ways I can bring AHI to zero or closer to zero. Should I use a nose dilator: internal or external? In the past, I had used BreatheRite without much benefit though about 10 years back when I did not have cpap machine, my room partner told it had reduced my choking and snoring. However, yesterday, when I used it with cpap, the AHI was 4.5. Would you suggest me to do this experiment for longer duration, say, a few weeks?
